Axis Talent Development, LLC d/b/a AxisTD, a Georgia Limited Liability Company, Referral Program General Disclaimer – Recipients of referral rewards are responsible for all tax liabilities, if any, on their rewards. Participants agree to abide by these rules and by the decisions of Axis Talent Development, LLC, which are final, binding and non-appealable, on all matters. Axis Talent Development, LLC reserves the right to disqualify any participant that it determines, in its sole discretion, is ineligible to participate in the program. Axis Talent Development, LLC is not responsible for lost, misdirected or delayed referrals. Upon Axis Talent Development, LLC's determination that you should receive a cash referral bonus, Axis Talent Development, LLC will contact you to request your W-9. Your W-9 must be submitted and received by Axis Talent Development, LLC, or your referral bonus will be held until Axis Talent Development, LLC receives your completed W-9. You may also choose to receive your referral bonus as a credit to your company's Axis Talent Development, LLC invoice (if your company is a current Axis Talent Development, LLC client), or you may choose to donate your referral bonus to any one of several participating charities. Axis Talent Development, LLC reserves the right to rescind and/or donate your referral bonus to any charity of its choosing if no response is received within 60 days from when Axis Talent Development, LLC contacted you for your W-9 and bonus payment preference. To the maximum extent permitted by applicable law, Axis Talent Development, LLC reserves the right to change or cancel the program at any time, at its sole discretion.
